“Just to let you know, with your permission we’ll be audio recording today’s session, and all future sessions, just so we can write up your notes accurately afterwards. It’s audio only, no video, and the recording is deleted as soon as that’s done. Are you happy for us to go ahead?”
If they say no:
“No problem at all. I’ll make my notes after the session instead. It won’t change anything.”
